It is initiated by the German National Research Data Infrastructure for Personal Health Data (nfdi4health), a consortium that plans to establish a concept for a federated research data infrastructure for personal health data in Germany.

This document contains the field questionnaires used in the CILS4COVID survey. Since the CILS4COVID survey included a survey experiment, there are two versions of the questionnaire that only differ in their question order. Both versions of the postal questionnaire are presented separately in the document. The web questionnaire contains both versions and documents the filters used to assign participants to one of the two experimental groups.
